Skilled Nursing Services

Rosner Home Health Care, Inc.’s nurses provide professional, yet gentle care to their patients in the privacy of their home.  An individualized plan of care is developed in coordination with the patient’s physician based on the patient’s specific or specialized needs. Services are only provided if they are medically reasonable and necessary for the patient’s diagnoses and for treatment of the illness or injury.

Skilled nursing services include:

  • Assessment and education regarding home safety
  • Assessment and education regarding disease processes

  • Medication education

  • Pain management

  • Wound care

  • Infusion but excluding blood/blood products

  • Ostomy care

  • Injections

Physical Therapy Services

In cooperation with the patient’s physician, the physical therapists from Rosner Home Health Care, Inc. provide an individualized plan of care to meet the patient’s specific need. The goal is for the patient to achieve the highest level of function while improving range of motion and strength including pain relief and prevention from disability resulting from disease, injury or loss of to a part of the body. Physical therapy is provided by a licensed physical therapist (PT) or by a licensed physical therapy assistant under the direction of a licensed PT.

  • Assessment and instruction on home safety

  • Assessment of equipment needs

  • Home exercise programs

  • Pain management

  • Gait training

  • Strengthening of extremities

  • Transfer training

  • Ultrasound

Occupational Therapy Services

The goal in occupational therapy is to improve a patient’s ability to perform the tasks needed for daily living. These include eating, grooming, bathing, toileting, dressing, and walking/ambulating. Occupational therapists often are able to recommend helpful equipment (such as grab bars, bath benches, etc.) in order to improve and restore functions impaired by either illness or injury, and make suggestions for home safety.

Occupational therapy is provided by a licensed occupational therapist (OT) or by a certified occupational therapy assistant (COTA) under the direction of a licensed OT. Occupational therapy in the home setting is generally provided in coordination with physical therapy or nursing services.

Occupational therapy services include:

  • Home exercise program

  • Activities of daily living

  • Strengthening of upper extremities

  • Homemaking tasks such as cooking and washing dishes

  • Handwriting

  • Fabrication of splints

  • Assessment of equipment needs

  • Ultrasound

Speech Therapy Services

Speech therapy services are provided to treat speech and language disorders that result in communication disabilities. The services are also provided to treat swallowing disorders (dysphagia), regardless of the presence of a communication disability.

Speech therapy services include:

  • Home exercise program

  • Swallowing techniques

  • Stroke rehab

  • Cognitive functioning

  • Memory functioning

  • Voice disturbances

Home Health Aide Services

Home Health Aide services provided by Rosner Home Health Care, Inc. are generally in coordination, supervision and support of Skilled Nursing or Physical Therapy. Home health aide services are centered around a traditional bath and help maintain a patient’s health and facilitate treatment of their illness or injury.  All Home health aides of Rosner Home Health Care, Inc. are Certified Nursing Assistants.

Home Health Aide services include:

  • Assisting with home exercise program as directed by a therapist
  • Preparation of light meals
  • Bath and personal care
  • Cleaning work area
  • Hair and nail care

Dieticians

Rosner Home Health Care, Inc. is associated with dietitians that help its patients how to prevent and treat illnesses by promoting healthy eating habits.  They evaluate needs of the patient then develop individualized nutritional care plans while instructing the patient’s family on proper administration of the care plan and recommending dietary modifications as needed.  Examples of dietary modification might be as simple as using less salt when preparing meals for those who have high blood pressure or reducing fat and sugar content of meals for those who have weight problems.

Medical Social Worker

On as needed basis, Rosner Home Health Care, Inc. may recommend its patient to a licensed Medical Social Worker who will assist and counsel the individual and his/her family with health related financial, social, and emotional concerns according to the physician’s orders.
